چکیده

A 39-year-old white man with acute rheumatic fever at the age of 19 years remained asymptomatic until 3 years before demise, when he started to have fainting episodes associated with chest pain and one year later symptoms of congestive heart failure. Examination at the Ohio State University Hospital showed signs of aortic stenosis, and cardiac catheterization revealed a pressure gradient of 90 mm. water across the aortic valve. He underwent open-heart surgery for the placement of a Starr-Edwards prosthetic valve in the aortic ostium. The postoperative course was complicated by anemia and wound infection with a coagulasepositive Staphylococcus but he responded to treatment fairly well. Four months after the operation the patient developed fatigue, occasional fever, and chills. One week before his last admission he noted petechiae, dyspnea, and orthopnea. Five months after the operation he was again admitted to University Hospital. On physical examination at this admission the patient was slightly dyspneic, the temperature was 100.2 F., and the blood pressure was 112/40. The neck veins were minimally distended at 300 position. The lungs revealed bilateral rales, particularly in the bases. The heart rate was regular at 87 per minute. The point of maximal impulse was in the sixth intercostal space at the left anterior axillary line. There were a grade-V/'VI holosystolic regurgitant murmur throughout the precordium radiating to the left axilla, a gradeIII/VI systolic ejection murmur in the right second intercostal space radiating toward the

Mycotic aortic aneurysm as a postsurgical complication: report of a case and review of the literature

Mycotic aneurysms are localized and irreversible dilatations of the arteries caused by weakening and damaging the arterial wall by an invasive organism establishing infective arteritis. Mycotic aneurysm of the thoracic aorta is a rare event; however, it can be fatal if not diagnosed early or not treated appropriately. Clinical findings are usually nonspecific; however, c...

Aortic Valve replacement with Reduction Aortoplasty with Mesh Prosthesis in 75 years old with Sever aortic Regurgitation with Aortic Aneurysm

Although classic and standard procedure for patients with ascending Aorta aneurysm with Sever aortic Regurgitation is Bentall surgery but in some selected cases this complex surgery was not performed with different reason like advanced age and coexisting comorbidity involving renal and hepatic and lung Function. In our cases 75 years old man with chronic renal failure ( Cr= 3.2 ) who was known ...

Delayed Infective Endocarditis with Mycotic Aneurysm Rupture below the Mechanical Valved Conduit after the Bentall Procedure.

UNLABELLED The Bentall procedure is the gold standard for treating aortic dissection complicated with valvular and ascending aorta disease. Recent results for this procedure have been excellent; nearly 100% of patients remain free of infective endocarditis in long-term follow-up. We report a case of delayed Streptococcus agalactiae infective endocarditis complicated by mycotic aneurysm in a man...

Mycotic aneurysm of the abdominal aorta.

Mycotic aneurysm of the abdominal aorta is an uncommon disease that carries a high mortality rate. In this report, two patients with this disease are presented. In the first case, Salmonella sp was cultured from an atherosclerotic aneurysm, and in the second patient, the aneurysm was a complication of Staphylococcus aureus bacterial endocarditis. Both presented suggestive clinical findings of t...

An Aortoenteric Fistula Arising after Endovascular Management of a Mycotic Abdominal Aortic Aneurysm Complicated with a Psoas Abscess

Mycotic aortic aneurysms account for 1–3% of all aortic aneurysms. The management of this disease is controversial. Since open surgical repair is associated with high morbidity and mortality rates, endovascular aneurysm repair is an alternative treatment method with promising early and midterm outcomes, although its long-term durability is unknown.Secondary aortoenteric fistulas may occur iatro...

Myocardial Infarction and Aortic Root Mycotic Aneurysm Complicating Aortic Valve Endocarditis: Utility of Cardiac CT

Aortic mycotic aneurysms are a rare but life-threatening potential complication of infective endocarditis. Rapid deterioration of the vascular wall in highly focal areas makes these pseudoaneurysms particularly prone to rupture, resulting in uncontrolled aortic hemorrhage. While computed tomography angiography (CTA) is the imaging modality of choice for the evaluation of mycotic aneurysms, it i...
